Training Enrollment Form

Click the link below to enroll in available training. At this time, only CHR I: Foundations is available. Once you submit your application, it will be reviewed and, if approved, you will receive information on how to access your course. Please note that the review process can take up to 10 days. You will receive periodic email updates about your status. Submitting multiple requests will delay approval—please submit only once.

To qualify for this training, you must meet all of the following requirements:

  • Be currently employed and serving as a Community Health Representative (CHR).
  • Work for an employer that is part of the IHS health system.
  • Have approval from your supervisor.
  • Submit a complete application with accurate information, which will be verified before approval.

Course Link

Use the link below to go directly to your course. Please note that you must first enroll and be approved before accessing any course materials. Once approved, you will receive an email that contains a course link, username, and password. Your username will be the email address that you provide in the enrollment form. If you have lost your password, you can use the “Forgot Password” option available on the sign-in page, accessible via the link below. Please see help videos and FAQs below if you have any questions.

How to Reset or Change Your Password

Use the instructions below to reset a forgotten password or change your current Blackboard password.

Reset a Forgotten Password

1. Go to the Blackboard login page.

2. Select Forgot Password?

Select Forgot Password

3. Choose one of the available recovery options:

  • Email Address: Enter your first name, last name, and the email address associated with your Blackboard account.

4. Select Submit.

Submit

5. Check your email for a password-reset message and follow the instructions provided.
6. Create a new password, confirm it, and submit the change.
7. Return to the Blackboard login page and sign in using your new password.

The reset link may expire, so use it as soon as possible. If you do not receive the email, check your spam or junk folder before contacting support.

Change Your Current Password

If you know your current password and are already signed in:

  1. Sign in to Blackboard.
  2. Select your name from the navigation menu to open your Profile.
  3. Locate Password or Change Password under your account or system settings.
  4. Enter your current password.
  5. Enter and confirm your new password.
  6. Select Save or Submit.

The option to change your password may not appear if your organization manages Blackboard accounts through another system or uses single sign-on.

Password Tips

Your new password should:

  • Meet the password requirements displayed on the screen
  • Be different from your previous passwords
  • Avoid personal information that others could easily guess
  • Be used only for this account

  • You no longer have access to the email address associated with your account
  • Your account becomes locked
  • You receive an error when attempting to reset or change your password

The Lesson Won’t Open

The most likely explanation include:
  • An internet disruption at the time of accessing the training
  • Your popup blocker is stopping the lesson from launching
  • Or you are using an outdated browser.
Please give it another try using the Chrome browser and check that you have a strong internet connection when accessing.  
If you continue to experience difficulties, it may be because of a pop-up blocker.  
Here are steps to disable your pop-up blocker.
Steps to Disable Pop-up Blocker (Desktop): 
  1. Open Chrome and click the three-dot menu (top right). 
  2. Select Settings
  3. Navigate to Privacy and security in the left menu. 
  4. Click Site Settings
  5. Scroll down and click Pop-ups and redirects
  6. Under “Default behavior,” select Sites can send pop-ups and use redirects
